About Us

Built around what families actually need.

Connected Home Health Care was founded on a simple idea: home care should keep families informed, treat caregivers with respect, and put the person being cared for at the center of every decision.

Our Story

Where Connected Home Health began.

Mary Kaba founded Connected Home Health Care because she believes home care should be more than just a service — it should be a real connection between caregivers, clients, and the families who love them.

After watching too many families struggle to find consistent caregivers and stay informed about the care their loved ones were receiving, Mary set out to build something different. Her vision was straightforward: an agency where caregiver consistency, open communication, and genuine compassion are the standard on every shift — not the exception. Today, Mary leads a team of caregivers across New Jersey who share that same commitment, treating every client like family because that is the kind of care every senior deserves.

What We Stand For

Three commitments that guide every decision.

Treat each client like family

Every care plan starts with the question: how would we want our own parent treated? That standard guides everything.

Keep families connected

Communication is not a value-add. It’s the work. Families should never be wondering how their loved one is doing.

Invest in our caregivers

Caregivers who are paid fairly, trained well, and treated with respect provide better care. It’s that simple.

Our Caregivers

The people who will be in your home.

Hiring is the most consequential decision an agency makes. We don’t cut corners on it.

Carefully screened

Every caregiver passes background checks, reference verification, and an in-person interview. We turn down more applicants than we hire.

Trained for the work

Caregivers complete training in personal care, safe transfers, infection control, and dementia care basics — plus specialty training as needed.

Matched for fit

We consider personality, language, schedule, and the kind of company your loved one will actually enjoy. The right match makes everything else easier.

Supported on the job

Caregivers have a real office team behind them — supervision, supplies, training, and someone to call when questions come up.

Care You Can See

Moments that define the work.

Compassion, patience, presence — the things that turn home care into something families can actually feel.

Walking outside together
Companionship and time together
Holding hands — a moment of care
